OSHA cites Mansfield steel supply plant after fatal accident- The 20th China (Guangzhou ) Int’l tube & pipe Processing Equipment Exhibition

The US Department of Labor''s Occupational Safety and Health Administration has cited National Material Company LLC for failing to protect workers from tip over hazard after an employee suffered fatal injuries at the steel supply plant in Mansfield. OSHA cited the steel supplier for one willful violation and proposed the maximum penalty allowed by law of USD 129, 336.
On February 23, 2018, Mr Andrew Cooper, 56, was working with a large roll of steel when investigators say that six steel coils weighing approximately 1,775 pounds each struck the employee after they tipped while being moved. He died of the injuries sustained in that accident, according to the Richland County Coroner''s Office.
Investigators said that three other instances of coils tipping over have occurred at that plant in the past two years. No injuries were reported from those incidents.
OSHA Toledo Office Director Kimberly Nelson said that "Employers are required to ensure safe work procedures in their facilities. This tragedy could have been avoided if the company had changed its coil transfer procedures after experiencing this serious hazard in the recent past."
The company will have 15 days to comply, request a conference or contest the citation.
